RESUMO

Biofilms are responsible for the most prevalent oral infections such as caries, periodontal disease, and pulp and periapical lesions, which affect the quality of life of people. Antibiotics have been widely used to treat these conditions as therapeutic and prophylactic compounds. However, due to the emergence of microbial resistance to antibiotics, there is an urgent need to develop and evaluate new antimicrobial agents. This scoping review offers an extensive and detailed synthesis of the potential role of selenium nanoparticles (SeNPs) in combating oral pathogens responsible for causing infectious diseases. A systematic search was conducted up until May 2022, encompassing the MEDLINE, Embase, Scopus, and Lilacs databases. We included studies focused on evaluating the antimicrobial efficacy of SeNPs on planktonic and biofilm forms and their side effects in in vitro studies. The selection process and data extraction were carried out by two researchers independently. A qualitative synthesis of the results was performed. A total of twenty-two articles were considered eligible for this scoping review. Most of the studies reported relevant antimicrobial efficacy against C. albicans, S. mutans, E. faecalis, and P. gingivalis, as well as effective antioxidant activity and limited toxicity. Further research is mandatory to critically assess the effectiveness of this alternative treatment in ex vivo and in vivo settings, with detailed information about SeNPs concentrations employed, their physicochemical properties, and the experimental conditions to provide enough evidence to address the construction and development of well-designed and safe protocols.

Background: Post-endodontic pain (PEP) management is an important factor to be considered in endodontic treatment. Several risk factors have been described that can attribute to its appearance. Laser-assisted disinfection has been described by many authors for its antimicrobial effect. Few studies described the relation between laser disinfection and its effect on PEP. The objective of this review is to describe the relation between different intracanal laser disinfection techniques and their effects on PEP. Material and Methods: An electronic search strategy was performed in Pubmed, Embase, and Web of Science (WOS) databases without restrictions as to the date of publication. Eligibility criteria were randomized controlled clinical trials (RCT) that used one of the different intracanal laser disinfection techniques in their experimental groups evaluating PEP outcome were included. Risk of bias analysis was performed by the Cochrane risk of bias tool. Results: The initial research identified 245 articles from which 221 were excluded and 21 studies were sought for retrieval and 12 articles met our inclusion criteria for the final qualitative analysis. The laser systems used were Nd:YAG, Er:YAG and, diode lasers including photodynamic therapy. Conclusions: The diode lasers showed the most promising results in terms of PEP reduction while Er:YAG showed more short-term efficacy (6 hours postoperative interval). The variables could not be analyzed homogenously due to the differences in the study designs. More RCT are needed comparing different laser disinfection techniques with the same baseline endodontic pathology to establish a specific protocol for the best outcome. Background: Diode laser (DL) can be used in endodontics both for its bactericidal effect inside the root canal system (RCS) and for photobiomodulation therapy (PBMT) to accelerate the repair of periradicular bone tissue. Clinical Cases: This work presents two cases of pulp necrosis/asymptomatic apical periodontitis (AAP) that were treated with 940-nm DL, administered both to disinfect the root canal and to apply PBMT to the periradicular tissues. The cases were analysed by Cone-Beam Computed Tomography (CBCT). Discussion: DL has become widely accepted due to its high antimicrobial effectiveness and its ability to accelerate the repair of large apical lesions by biostimulation. Nevertheless, differences of opinion persist within the scientific community due to the lack of standardized endodontic protocols. Conclusions: The application of 940-nm DL, both for disinfection of the RCS and for PBMT, is an effective treatment in non-vital teeth with large periapical lesions. In both cases reported, bone neoformation were found at the 6-month check-up. Key words:Low-level laser therapy, photobiomodulation therapy, diode laser, endodontics.

SUMMARY: The aim of this of this study was to evaluate the prevalence and morphology of radix entomolaris (RE) in the mandibular first molar (MFM) in a southern Chilean sub-population by cone-beam computed tomography (CBCT). Two hundred ten CBCT images of MFM were analysed. To detect the presence of RE, the observation and measurements were standardised, advancing 1 mm to apical from the floor of the pulp chamber to the most apical zone of the tooth. The data were analyzed using descriptive statistics, with a value of P < 0.05 being statistically significant. The prevalence of a third root in the permanent MFM was 5.7 % (12/210). The incidence of RE was the same in male patients (2.9 %) as in female patients (2.9 %) (p=0.324). The occurrence of an RE in the left MFM was 7.3 % (3/41 patients) for male patients and 4.5 % (3/66) for female patients (p=0.398). The right MFM in male patients had an incidence of 8.1 % (3/37) and 4.5 % (3/66) for female patients (p=0.340). When the presence of an RE was compared between the left (5.6 %) and right side (5.8 %), no statistically significant differences were found (p=0.998). The frequency of bilateral three-rooted MFM was 71.43 % (5/7) and 28.57 % (2/7) were unilateral. The prevalence of RE in MFM in a southern Chilean sub-population was 5.7 %. The occurrence was bilateral in the majority of cases and no significant differences were observed by sex or side. The commonest type of RE was Type 1 (83.3 %). The use of CBCT can improve the endodontic treatment outcome.


RESUMEN: El objetivo de este estudio fue evaluar la prevalencia y morfología de radix entomolaris (RE) en el primer mo- lar mandibular (MFM) en una subpoblación del sur de Chile mediante tomografía computarizada de haz cónico (CBCT). Se analizaron doscientas diez imágenes CBCT de MFM. Para detectar la presencia de OD se estandarizó la observación y las medidas, avanzando 1 mm apical desde el suelo de la cámara pulpar hasta la zona más apical del diente. Los datos fueron analizados mediante estadística descriptiva, siendo estadísticamente significativo un valor de P < 0,05. La prevalencia de una tercera raíz en el MFM permanente fue de 5,7 % (12/210). La incidencia de ER fue la misma en pacientes masculinos (2,9 %) que en pacientes femeninas (2,9 %) (p=0,324). La aparición de un ER en el MFM izquier- do fue del 7,3 % (3/41 pacientes) para los pacientes masculinos y del 4,5 % (3/66) para las pacientes femeninas (p=0,398). El MFM derecho en pacientes masculinos tuvo una incidencia de 8,1 % (3/ 37) y 4,5 % (3/66) para pacientes femeninos (p=0,340). Cuando se comparó la presencia de un OD entre el lado izquierdo (5,6 %) y derecho (5,8 %), no se encontraron diferencias estadísticamente significativas (p=0,998). La frecuencia de MFM de tres raíces bilaterales fue 71,43 % (5/7) y 28,57 % (2/7) fueron unilaterales. La prevalencia de RE en MFM en una subpoblación del sur de Chile fue de 5,7 %. La presentación fue bilateral en la mayoría de los casos y no se observaron diferencias significativas por sexo o lado. El tipo de ER más frecuente fue el tipo 1 (83,3 %). El uso de CBCT puede mejorar el resultado del tratamiento endodóntico.

SUMMARY: The maxillary first premolar (MFP) presents a highly variable and complex root morphology; it is also one of the teeth most likely to suffer vertical root fractures. The aim of this study was to describe the morphology of the root and root canal system of the MFP in a Chilean population using Cone-Beam Computed Tomography (CBCT). One hundred and twenty-one MFP were evaluated (60 left and 61 right), belonging to subjects of both sexes. Descriptive and statiscal analysis of the data was carried out, with a value of P < 0.05 being statistically significant. One root was observed in 71 % of teeth and two roots in 29 %; statistically significant differences were observed in the number of roots by sex (p<0.05). Two root canals were found in 69.5 % of teeth and one canal in 30.5 %. In terms of morphological classification, 41.3 % of cases were Vertucci Type IV. The proximal cementodentinal walls were the thinnest. The MFP presented a high degree of morphological variation in a Chilean subpopulation.


RESUMEN: El primer premolar maxilar (PPM) presenta una morfología radicular muy variable y compleja; también es uno de los dientes con mayor probabilidad de sufrir fracturas radiculares verticales. El objetivo de este estudio fue describir la morfología de la raíz y el sistema de conductos radiculares del PPM en una población chilena mediante tomografía computarizada de haz cónico (CBCT). Se evaluaron 121 PPM (60 izquierdas y 61 derechas), pertenecientes a sujetos de ambos sexos. Se realizó un análisis descriptivo y estadístico de los datos, siendo estadísticamente significativo un valor de P < 0,05. Se observó una raíz en el 71 % de los dientes y dos raíces en el 29 %; se observaron diferencias estadísticamente significativas en el número de raíces por sexo (p<0.05). Se encontraron dos conductos radiculares en el 69,5 % de los dientes y un conducto en el 30,5 %. En cuanto a la clasificación morfológica, el 41,3 % de los casos fueron Vertucci Tipo IV. Las paredes cementodentinales proximales fueron las más delgadas. El PPM presentó un alto grado de variación morfológica en una subpoblación chilena.

The eradication of endodontic pathogens continues to be the focus of the search for new root canal system (RCS) disinfection strategies. This scoping review provides a comprehensive synthesis of antimicrobial photodynamic therapy (aPDT) using nanoparticles (NPs) as an alternative to optimize RCS disinfection. A systematic search up to March 2021 was carried out using the MEDLINE, EMBASE, Scopus, Lilacs, Central Cochrane Library, and BBO databases. We included studies focused on evaluating the activation of NPs by aPDT in inoculated root canals of human or animal teeth or bacterial cultures in the laboratory. The selection process and data extraction were carried out by two researchers independently. A qualitative synthesis of the results was performed. A total of seventeen studies were included, of which twelve showed a substantial antibacterial efficacy, two assessed the substantivity of the disinfection effect, and three showed low cytotoxicity. No adverse effects were reported. The use of functionalized NPs with photosensitizer molecules in aPDT has been shown to be effective in reducing the bacteria count, making it a promising alternative in endodontic disinfection. Further studies are needed to assess the development of this therapy in in vivo conditions, with detailed information about the laser parameters used to allow the development of safe and standardized protocols.

BACKGROUND: Crown discoloration is an undesirable side effect of the use of triple antibiotic paste (TAP) in regenerative endodontic procedures (REPs). The aim of this ex vivo study was to assess the potential for tooth discoloration and induction of fluorescence associated with the use of TAP containing either doxycycline (DOX) or clindamycin (CLIN), and of calcium hydroxide (Ca(OH)2), by spectrophotometric analysis and confocal laser scanning microscopy (CLSM). MATERIAL AND METHODS: A total of forty single-rooted human teeth extracted by therapeutic indication were used. The root canals were enlarged using the step-back technique up to a K #80 file and were randomly divided into four experimental groups (10 specimens each): i)Ca(OH)2 group, ii)TAP with DOX group, iii) TAP with CLIN group, iv) Control group (no treatment). To quantify the change of color of the different groups of teeth included, the Vita Easyshade advance 4.0 spectrophotometer was used. CLSM was used to determine fluorescence. RESULTS: After 28 days of action inside the root canal, no extreme discoloration was visible, to the naked eye, in any of the teeth included in the study. Under the conditions of this ex vivo study, TAP with DOX induced the highest crown discoloration among the medicaments tested. In contrast, Ca(OH)2 and TAP with CLIN did not induce crown discoloration after 28 days. The TAP with DOX group presented the highest fluorescence measurements. CONCLUSIONS: Considering the discoloration potential and fluorescence changes in TAP with DOX or other tetracyclines, and the cytotoxic effect of TAPs, we recommend the use of Ca(OH)2 for REPs. RESUMEN: El objetivo de la endodoncia es prevenir o curar la periodontitis apical. Por lo tanto, los microorganismos que han colonizado el sistema de canales radiculares deben ser eliminados para promover la regeneración y cicatrización. Desafortunadamente, la existencia de canales accesorios, anastomosis, istmos, así como ramificaciones apicales, genera una compleja red tridimensional en el interior de la raíz, lo que dificulta la eliminación total de bacterias y detritus. La te rapia endodóntica convencional utilizada actualmente presenta una serie de limitaciones.Se ha demostrado que los instrumentos endodónticos dejan un 35 % o más de superficie dentinaria sin tratar. Por otro lado, la imposibilidad de generar un flujo turbulento en el interior de los canales, impide que el irrigante llegue a zonas de difícil acceso. Esto facilita la persistenc ia de biopelículas bacterianas y la supervivencia de un número importante de bacterias viables. La irrigación activada por láser (LAI) ha sido propuesta como una tecnología co-adyuvante a la terapia quimio-mecánica para optimizarla limpieza y desinfección. Los láseresde Er,Cr:YSGG (2780nm) y Er:YAG (2940nm) son los más utilizados. Estas dos longitudes de onda son capaces de ser absorbidas ampliamente por diferentes soluciones irrigantes, teniendo en cuenta un uso seguro y respetando los parámetros clínicos aceptados. La absorción de la energía del láser por parte de los irrigantes, genera burbujas de vapor en el interior del fluido, que liberan fuerzas de cizalla al implosionar. Este fenómeno, denominado cavitación, genera mayor limpieza y desinfección del interior del sistema de canales radiculares, incluso en zonas de difícil acceso. Diversos estudios microbiológicos y microscópicos, han demostrado la efectividad antibacteriana de LAI. Recientemente, se ha publicado que LAI tendría la capacidad de incrementarla capacidad antibacteriana del hipoclorito de sodio a baja concentración (un buen desinfectante pero extraordinariamente tóxico), lo que permitiría trabajar con concentraciones menores y por lo tanto más seguras para el paciente.


ABSTRACT: The goal of endodontics is to prevent or cure apical periodontitis. Therefore, microorganisms that have colonized the root canal system must be eliminated to promote regeneration and healing. Unfortunately, the existence of accessory canals, anastomoses, isthmus, as well as apical ramifications, form a complex three-dimensional network inside the root, which makes difficult the total elimination of bacteria and detritus. Conventional endodontic therapy has several limitations. Endodontic instruments have been shown to leave 35 % or more of the dentin surface untreated. On the other hand, the inability of generating a turbulent flow, prevents the irrigant from reaching areas that are difficult to access. This facilitates the persistence of bacterial biofilms and the survival of a significant number of viable bacteria. Laser-activated Irrigation (LAI) has been proposed as a co-adjuvant to chemo-mechanical therapy to improve cleaning and disinfection. Er, Cr: YSGG (2780nm) and Er: YAG (2940nm) lasers are the most widely used. These two wavelengths are capable of being widely absorbed by different irrigating solutions, taking into account safe use and respecting accepted clinical parameters.The deep absorption of laser energy generates steam bubbles inside the fluid, which release shear forces when imploding. This phenomenon, called cavitation, is responsible for greater cleaning and disinfection within the root canal system, even in areas difficult to access. Several studies have demonstrated the antibacterial effectiveness of LAI through microbiological methods and microscopic techniques. Recently, it has been reported that LAI would have the ability to increase the antibacterial capacity of low concentration of sodium hypochlorite (a good disinfectant but extremely toxic), which would allow working with concentrations that are safer for the patient.

SUMMARY: The root canal system of the maxillary first premolar (MFP), present a high rate of variation, especially at apical level. This complicates the action of antiseptic solutions and endodontic instruments at this level. The object of this in vivo study was to analyse by Cone Beam Computed Tomography (CBCT) the radicular and canalicular morphology of MFP in a Chilean sub-population. We carried out a cross sectional, descriptive and observational in vivo study with CBCT examinations of 70 MFP, both left and right. The data were analysed using descriptive statistics (mean (M), standard deviation (SD), with a confidence interval of 95 %), and one-factor ANOVA was used to relate the sections observed. Tooth 1.4 presented one root in 64.86 % of cases and two roots in 35.15 %. Tooth 2.4 presented one root in 66.67 % of cases and two roots in 33.33 %. The frequency of one and two canals was observed to be 30 % and 70% respectively. The walls with the smallest cementodentinal thickness were the mesial (1.11±0.55) and distal (1.08±0.52). The thickest dentinal wall was the palatine (2.07±1.29), followed by the buccal (1.6±1.0). No statistical differences between males and females were found in the thickness of the root wall, nor in the location of the premolar in the maxilla (p>0.05). In conclusion, the root morphology of the MFP is highly variable. Care must be taken not to over-instrument the proximal walls to avoid perforations or fractures. CBCT has proved to be a useful and effective diagnostic tool for in vivo study of dental morphology.


RESUMEN: El sistema de canal radicular del primer premolar maxilar (MFP) presenta una alta tasa de variación, especialmente a nivel apical. Esto complica la acción de las soluciones antisépticas y los instrumentos endodónticos a este nivel. El objetivo de este estudio in vivo fue analizar mediante tomografía computarizada de haz cónico (CBCT) la morfología radicular y canalicular de la MFP en una subpoblación chilena. Realizamos un estudio transversal, descriptivo y observacional in vivo con exámenes CBCT de 70 MFP, tanto a la izquierda como a la derecha. Los datos se analizaron mediante estadística descriptiva (media (M), desviación estándar (DE), con un intervalo de confianza del 95 %) y se utilizó ANOVA de un factor para relacionar las secciones observadas. El diente 1.4 presentó una raíz en el 64,86% de los casos y dos raíces en el 35,15 %. El diente 2.4 presentó una raíz en el 66,67 % de los casos y dos raíces en el 33,33 %. Se observó que la frecuencia de uno y dos canales era del 30 % y 70%, respectivamente. Las paredes con el espesor cementodentinal más pequeño fueron mesial (1,11 ± 0,55) y distal (1,08 ± 0,52). La pared dentinaria más gruesa fue la palatina (2,07 ± 1,29), seguida de la vestibular (1,6 ± 1,0). No se encontraron diferencias estadísticas entre machos y hembras en el grosor de la pared de la raíz, ni en la ubicación del premolar en el maxilar (p> 0,05). En conclusión, la morfología de la raíz de la MFP es muy variable. Se debe tener cuidado de no sobre-instrumentar las paredes proximales para evitar perforaciones o fracturas. CBCT ha demostrado ser una herramienta de diagnóstico útil y efectiva para el estudio in vivo de la morfología dental.

Objective: To compare the antibacterial effectiveness of 0.5% sodium hypochlorite (NaOCl) activated by the Er,Cr:YSGG laser-activated irrigation (LAI) and passive ultrasonic irrigation (PUI) against a 10-day-old intracanal Enterococcus faecalis biofilm. Background: LAI and PUI are regarded as alternative methods to release the irrigant in the inner regions of the root canal system achieving enhanced cleaning ability. Nevertheless, little evidence regarding the activation of low concentrations of NaOCl has been reported. Materials and methods: Seventy-two single-rooted teeth were instrumented, inoculated (E. faecalis ATCC 29212), and incubated for 10 days to allow biofilm formation. Specimens were randomly divided into six groups (n = 12 each): (1) 0.5% NaOCl+Er,Cr:YSGG LAI, (2) saline+Er,Cr:YSGG LAI, (3) 0.5% NaOCl+PUI, (4) saline+PUI, (5) positive control (no treatment), and (6) negative control (no bacteria). The activation time was distributed as follows: 30 sec of activation, followed by a rest phase of 30 sec, and ending with 30 sec of activation. The number of bacterial survivors was determined by plate counting. Results: Both irrigation regimens LAI and PUI reduced the number of colony-forming unit. Moreover, LAI +0.5% NaOCl and the rest of groups significantly differ (p < 0.001 for all comparisons). Conclusions: Er,Cr:YSGG LAI proved to be more effective than PUI in enhancing the antimicrobial activity of 0.5% NaOCl against 10-day-old intracanal E. faecalis biofilms.

The onset and persistence of endodontic infections due to residual biofilm after chemical disinfection promotes secondary bacterial infection. Alternative methods to disinfect operated root canals are a matter of great interest. The aim was to evaluate the antibacterial effectiveness of sodium hypochlorite (NaOCl) at low concentrations activated by the Er,Cr:YSGG laser-activated irrigation (LAI) against 10-day-old intracanal Enterococcus faecalis biofilm. Biofilms were formed inside the root canals and divided into 7 groups (n13): 0.5% NaOCl + Er,Cr:YSGG; Saline + Er,Cr:YSGG; 0.5% NaOCl + syringe irrigation(SI); 2.5% NaOCl + SI; 5% NaOCl + SI; positive and negative controls. Bacterial survivors were counted and specimens visualized under scanning electron and confocal laser scanning microscopy. Treatments with 0.5% NaOCl + Er,Cr:YSGG and 2.5% NaOCl + SI gave a significant reduction in the number of CFU/mm2. Moreover, scanning electron microscopy and confocal laser scanning microscopy imaging confirmed and reinforced bacteriological data. Thus, Er,Cr:YSGG LAI proved to be able to improve the intracanal distribution of 0.5% NaOCl after 60 s of activation, reaching the same level of effectiveness than 2.5% NaOCl. This is regarded as of clinical interest, since working with lower concentrations may contribute to reduce undesired effects.

ABSTRACT: The apical limit of tooth roots for instrumentation and obturation is a controversial subject due to the high morphological complexity presented by the apical zone of the root. The development of electronic apex locators (EALs) has made working length (WL) determination more predictable, producing more accurate results; however, the interpretation mechanisms of different devices may affect measurements taken under different clinical conditions. One hundred premolars were used to compare the effectiveness of Propex II, Raypex 6, Propex Pixi and Root ZX II in locating the apical foramen (AF). No statistically significant differences were observed when the accuracy of measurement was compared between the different groups of EALs, however Root ZX II and Raypex 6 presented the best overall performance.


RESUMEN: El límite apical radicular para la instrumentación y obturación es un tema controversial, debido a la alta complejidad morfológica que presenta la zona apical de la raíz. El desarrollo de localizadores de ápice electrónicos (EALs) ha hecho la determinación de la longitud de trabajo (WL) más predecible y con resultados más precisos. Sin embargo, el mecanismo de interpretación de cada dispositivo puede afectar la determinación de las mediciones en diferentes condiciones clínicas. Cien premolares fueron utilizados para comparar la efectividad de Propex II, Raypex 6, Propex Pixi y Root ZX II en localizar el foramen apical (AF). No se observaron diferencias estadísticamente significativas cuando se comparó la precisión de medición entre los diferentes grupos de EALs, sin embargo, Root ZX II y Raypex 6 mostraron un mejor desempeño global.

SUMMARY: It is critical to know the precise location and configuration of the mandibular canal (MC) to avoid damages in the alveolar inferior nerve during an invasive dental treatment. The aim of this study was to provide anatomic relationships of the mandibular canal in its entire pathway related to cortical walls and dental roots to serve as basic knowledge for dental clinical procedures performed in this area. We analyzed 50 CBCT images assessing the specific location of MC at the retromolar space as related to inferior teeth roots. Data were analyzed by side using descriptive statistics (median: mean [M], standard deviation [SD], 95 % confidence interval: lower endpoint [IC] and upper endpoint [IC']). The MC length and pathway were measured from the mandibular foramen up to the mental foramen. MC was closer from lingual cortical than buccal at the retromolar space as well the distal root of the second molar and the root of the second premolar are closest to MC. A mean length for the MC of 70.8 (±5.3 mm) for the right side and 71 (±5.8 mm) for the left side were observed. In conclusion, it is critical for clinicians to know three-dimensionally the topographic relationships between the inferior teeth roots and the mandibular canal before proceeding to any invasive dental or surgical procedure at this region.


RESUMEN: Es crítico conocer la ubicación exacta y la configuración del canal mandibular (MC) para evitar daños en el nervio inferior alveolar durante un tratamiento dental invasivo. El objetivo de este estudio fue proporcionar relaciones anatómicas del MC en toda su trayectoria relacionada con paredes corticales y raíces dentales para servir como un conocimiento básico para los procedimientos clínicos dentales realizados en esta región. Se analizaron 50 imágenes CBCT evaluando la localización específica del MC en el espacio retromolar, así como las raíces de los dientes inferiores. Los datos se analizaron por lado, utilizando estadística descriptiva (mediana: media [M], desviación estándar [SD], 95 % intervalo de confianza: punto final inferior [IC] y extremo superior [IC]). La longitud y vía del conducto mandibular se midieron desde el foramen mandibular hasta el foramen mentoniano. El MC estaba más cerca de la cortical lingual que bucal en el espacio retromolar, así como la raíz distal del segundo molar y la raíz del segundo premolar están más cerca del MC. Se observó una longitud media para el MC de 70,8 (± 5,3 mm) para el lado derecho y de 71 (± 5,8 mm) para el lado izquierdo. En conclusión, es fundamental que el clínico conozca tridimensionalmente las relaciones topográficas entre las raíces de los dientes inferiores y el conducto mandibular antes de proceder a cualquier procedimiento invasivo dental o quirúrgico en esta región.

Abstract Case Description: The main objective of this paper was to report the clinical effectiveness of leukocyte- platelet- rich fibrin (L-PRF) in the treatment of a combined endo-periodontal lesion of an upper first premolar. Clinical Findings: The tooth had a profound abfraction on the vestibular aspect and presented no mobility but revealed a deep pocket measuring of 11 mm on the mesial vestibular aspect and 14 mm on the mesial palatine aspect. The three dimensional image analysis showed total bone loss in the mesial aspect and an extensively bone loss of the vestibular aspect of the vestibular root. Treatment and Outcome: Endodontic treatment was performed and periodontal access surgery (surgical periodontal therapy) was done with the application of autologous L-PRF. Three month and 6 months after surgery, the cone beam computed tomography (CBCT) exams showed no bone regeneration in any aspect of the tooth. However, periodontal examination showed a significative improvement in the deepness of surcus. The mesial vestibular aspect had a deep pocket of 3 mm and 5 mm on the mesial palatine aspect showing a reduction in deepness of 8 mm and 9 mm, respectively. Clinical Relevance: The actual treatment for teeth with bad prognosis is the extraction and replacement with implants. Even though implants are capable of restore function and aesthetic, the abuse of this approach have led to the loss of teeth that could be successfully treated with a less invasive technique. The prognosis of teeth with endoperiodontal lesion is poor but could be enhanced with regenerative therapies. Until now there are no clinical trials and just four case report about the treatment of these teeth with platelet rich fibrin.


Resumen Descripción del caso: El objetivo de este artículo fue reportar la efectividad clínica del uso de fibrina rica en plaquetas y leucocitos (L-PRF) en el tratamiento de una lesión endoperiodontal en un primer premolar superior. Hallazgos clínicos: El diente presentaba una abfracción profunda en la superficie vestibular, sin movilidad, pero reveló una bolsa profunda de 11 mm en la superficie mesio-vestibular y de 14 mm en la superficie palato-mesial. El análisis tridimensional de la imagen mostró una pérdida ósea total en la superficie mesial y una pérdida ósea extensa de la cara vestibular de la raíz. Tratamiento y resultado: Se realizó el tratamiento endodóntico y posteriormente cirugía de acceso periodontal (terapia periodontal quirúrgica) con la aplicación de L-PRF autóloga. Tres meses y 6 meses después de la cirugía, los examenes de Tomografía Computarizada Cone Beam (CBCT) no evidenciaron regeneración ósea en ningún sector del diente. Sin embargo, el examen clínico periodontal mostró una mejora significativa en la profundidad de la bolsa. BACKGROUND: It is very important for a clinician to know the increased wear of mechanized files when establishing endodontic therapy. The aim of this study was to check the wear of the Primary WaveOne file upon shaping two, four and six maxillary molar vestibular canals. MATERIAL AND METHODS: The deterioration of 40 files, divided into four groups, was evaluated microscopically: group 1, control (unused); group 2, two canals; group 3, four canals; and group 4, six canals. After instrumentation, the files were embedded in resin and sectioned at their apical third into three equal parts. To analyze the wear of edges in the different sections, AutoCAD software was used and analysis of variance (ANOVA) was then performed to compare the mean rake angles. RESULTS: The files with two and four uses showed slight wear, whereas those with six applications showed significant wear (p<0.05). CONCLUSIONS: Primary WaveOne files can be used in up to four root canals without their edges losing effectiveness. Key words:Files wear, reciprocating motion, shaping capacity, WaveOne.

Antecedentes: La Endocarditis infecciosa es una enfermedad grave y su incidencia ha aumentado en el último tiempo. En Chile faltan estudios observaciona-les que representen adecuadamente nuestra realidad local y nacional. Objetivo: Describir las características clínicas, ecográficas y bacteriológicas de los pacientes con Endocarditis Infecciosa del Hospital Carlos Van Buren de Valparaíso, entre los años 2012 y 2016. Métodos: Se realizó un análisis descriptivo de 35 pacientes con diagnóstico de Endocarditis infecciosa, recolectándose variables clínicas, imagenológicas y de laboratorio. Para describir los resultados se utilizó frecuencias, medianas y figuras. Resultados: Se observó un aumento de casos de Endocarditis infecciosa en los últimos dos años. Del total de pacientes con Endocarditis infecciosa, 28 casos (80%) eran de válvula nativa, siendo el compromiso más frecuente el de válvula aórtica. Se observó un aumento de las comorbilidades no cardíacas y de los casos provocados por el grupo Staphylococcus. Los síntomas más comunes fueron compromiso del estado general y fiebre, y las complicaciones más frecuentes fueron insuficiencia renal aguda, insuficiencia cardíaca aguda y fenómenos embólicos. Un total de 14 pacientes fallecieron producto de la infección, las complicaciones y cirugía. Conclusiones: Se realiza una caracterización actualizada de la enfermedad. Se necesitan más estudios con un mayor número de paciente para un mejor entendimiento de nuestra realidad.


Background: Infective endocarditis is a severe illness the incidence of which has increased over time. There are relatively few observational studies that adequately represent the characteristics of the disease in Chile. Aim: To describe clinical, echographic and bacteriological characteristics of patients with infective endocarditis at Carlos van Buren Hospital in Valparaíso, between 2012 and 2016. Methods: A descriptive analysis of 35 patients diagnosed with infective endocarditis was performed. Cinical, imaging findings and laboratory variables were collected. Relative frequencies, medians and figures were used to describe the results. Results: We observed an increase in cases of infective endocarditis in the last two years. Of all patients diagnosed with infective endocarditis, 80% had native valve involvement, more frequently at the aortic valve. There was an increase in non-cardiac comorbidities and Staphylococcus was identified as the main etiology. Weakness and fever were the most common symptoms, whereas more common complications were acute kidney and heart failure and embolic phenomena. A total of 14 patients died of infection, complications or after surgery surgery. Conclusions: An updated characterization of Infective endocarditis at a Chilean hospital is presen-ted. However, more studies are needed with a larger number of patients for a better characterization of the disease in our country.
